I hereby apply for membership in the Boston Druggists’ Association and agree that, if my application is approved, I will comply with the Bylaws and regulations of the Association.

Article II, Section 1. Any persons engaged in the wholesale, retail, hospital, sales, educational or other phases of pharmacy and related fields shall be eligible for membership in the BDA. Person desiring membership shall complete an application (see below) to be provided by the Secretary.
